Como todo proceder medico, las complicaciones asociadas
siempre van a estar presente. En la muestra, 9 pacientes
sufrieron de complicaciones, de ello 8 fueron complicaciones
transitorias y de menor complejidad (tabla No.8), Si
comparamos con la serie de Martín R, et al. 8 reportan
complicaciones intraoperatorias como la perforación del
esófago y la ruptura de la duramadre que fue solucionada
mientras que Herrero Vallejo, et al. 4 reportaba la
remoción del injerto óseo con compresión del
saco dural o por el contrario al esófago, por otro lado la
remoción de los tornillos también describe un
porciento considerables de pacientes. El daño de
estructuras vasculares o nerviosas es poco común en las
investigaciones consultadas.
En todos los casos se les realizo un seguimiento
postoperatorio en consulta especializada hasta el momento de su
alta que oscilo entre 12 meses hasta 36 meses, con un promedio de
26 meses con tomas periódicas de radiografías de la
columna cervical y extremando la evaluación clínica
ya que se contraindicaba el uso de otros medios de
imágenes. La tabla No 9 , compara
numéricamente la puntuación al antes de la
cirugía y al momento del alta medica definitiva
seleccionando la Escala modificada de la Asociación de
Ortopedia Japonesa(mJOA) las que permitió el uso de la
ecuación para evaluar la razón de
recuperación del paciente, en muestro grupo 14 pacientes
(63.6 %) presentó una recuperación por encima del
50% , tres paciente mostró mejoría parcial y en 4
pacientes, la cirugía impidió la progresión
de la enfermedad, de ahí que se consideren resultados
alentadores para la mielopatia espondilótica cervical por
medio de la corpectomia siempre se cumplan los criterios de
compresión anterior. La puntuación media
postoperatoria en la escala de la mJOA fue de 13.4, 3 puntos mas
que la preoperatoria, mientras que la media de la razón de
recuperación de los pacientes intervenidos fue de 44.2%
Para Martín y col 8 con la discectomia y fusión
intersomática por vía anterior para el tratamiento
de la mielopatia cervical son variables, con cifras de
mejoría en torno al 50 %, mientras que para Chilles, et
al. 25 se eleva el porciento hasta un 90, es posible como lo
plantea Martín R y col 8 en que estos dispares resultados
influya tanto la técnica quirúrgica – que se
consiga un mayor grado de descompresión del cordón
medular- como la selección de los pacientes. Desde el
punto de vista quirúrgico, las diferentes técnicas
de discectomia y fusión intersomática por
vía anterior resultan muy similares en cuanto a
concepción, pero muy diferentes en cuanto a su capacidad
para descomprimir las estructuras mielorradiculares. Mientras que
en la técnica de Cloward el espacio se encuentra limitado
al cilindro que talla el perforador en el espacio intervertebral,
en la corpectomia el fresado puede extenderse tanto lateralmente
como en el eje vertical hasta que el cirujano se encuentre
plenamente seguro de que la medula ha sido descomprimida en su
totalidad .Al analizar la durabilidad de la recuperación
funcional del paciente se corresponde con la serie del autor de
esta investigación en los primero 15 meses,
señalando Martín-Láez, et al. 8 en su
trabajo esto se ve comprometido pues comienza con la
reaparición de elementos degenerativos en los sitios
intervenidos dada la existencia de la muerte celular programada
en las áreas medulares afectadas, la descompresión
mielorradicular quirúrgica precoz parece abolir los
mecanismo etiopatogénicos que dan lugar a las cascadas
moleculares .Para Yonenobo, et al. 81 el abordaje vía
posterior crea una inestabilidad postlaminectomia y reporta en un
numero importante de los casos que la mejoría no sobrepasa
los primeros 10 meses del postoperatorio.

Conclusiones
El proceso de descripción llevado a cabo en la
presente investigación tuvo en cuenta que las
características demográficas de los pacientes que
sufrieron mielopatia espondilitica cervical demostrando que el
sexo masculino y mayores de 60 años fueron los más
afectados. El patrón mielopatico puro y dentro de este el
síndrome medular central fue la forma de
presentación en la mayoría de los casos.
De los segmentos cervicales que mayor compromiso
degenerativo al momento del diagnostico fue la vértebra
cervical quinta y la vértebra cervical sexta incluyendo el
ínterespacio correspondiente, se utilizo en todos los
casos las radiografías simples de columna cervical para
confirmar el diagnostico de espondilosis y apoyados en la
Tomografía axial computarizada. Los criterios de
selección de la técnica fue basada en la
confirmación por imágenes de barras degenerativas
espondilóticas por detrás de los cuerpos
vertebrales capaz de proporcionar un sufrimiento de los cordones
anterolaterales y de astas anteriores. El segmento de mayor
compromiso degenerativo se ubico entre la quinta y sexta
vértebra cervical por lo que la vertebrotomia
ocurrió en el cuerpo de C6 con discectomia de los espacios
inmediato superior e inferior, colocándose la
instrumentación en el bisegmento
correspondiente.
La corpectomia cervical como técnica de
elección en aquellos pacientes que cumplieron los
criterios quirúrgicos tuvo un elevado índice de
recuperación para la mayoría de los pacientes y
permitió en otros casos estabilizar el deterioro
neurológico en aquellos donde no ocurrió una
recuperación funcional. No se reportaron complicaciones
importantes de las estructuras neurales y/o paravertebrales
utilizando esta técnica quirúrgica.
Recomendaciones
Utilizar la corpectomia cervical en todos aquellos
pacientes que cumplan los criterios quirúrgicos para la
aplicación de esta técnica.
Profundizar por medios de investigaciones de tipo
prospectivo para definir las variables que influyen en la
evolución postquirúrgica de los pacientes
mielopaticos.
